Afoundria

Afoundria is a start-up company in the Health Information Technology arena. We deliver a suite of devices/applications/services that make the work of providers in the post-acute care (PAC) space more efficient and effective. Our EHR 1) is tightly integrated and interoperable with HIT functions, EMRs and PHRs, 2) provides sophisticated tools for clinical document creation, addressing complex requirements for content and integrated, real time, point-of-care decision support, 3) manages workflow effectively using universal, ubiquitous, secure communication tools and an elegant user interface, and 4) provides roll-up data for performing population care management. Further, we have embraced fully - from design through implementation - a patient-centric model of care and a team-based approach to promote, maintain, and restore health.
